CTE stage is not yet a timeline: bridging terminal pathology and functional decline

From the perspective of researchers studying neural injury, synaptic plasticity, and cognitive dysfunction, the central unresolved question is how a pathological stage measured at death should be connected to functional decline that began years earlier. In this study, CTE stage was determined only at autopsy, whereas dementia developed during life. Dementia began at a mean age of 63.4 years, while donors with stage IV CTE died at a mean age of 76.4 years. Thus, the analysis cannot establish whether stage IV pathology was already present when dementia began, whether pathology progressed from a lower stage during the interval between dementia onset and death, or whether both reflected a longer period of ageing and neurodegeneration. This distinction matters because CTE stage is itself strongly age dependent and is associated with survival after football exposure.[2] Adjustment for age at death cannot reconstruct this unobserved temporal sequence. A related issue is the biological gap between pathological burden and functional expression. Stage IV CTE may identify not only extensive CTE-related tau deposition but a broader state of neural network injury. Among donors with stage IV CTE, 77.9% had TDP-43 pathology, while vascular abnormalities, white matter rarefaction, Alzheimer-type changes, and Lewy body disease were also common.[1] Previous analyses of brain donors exposed to repetitive head impacts have shown that these pathologies are correlated and jointly contribute to dementia.[3] Cognitive impairment may therefore reflect the combined effects of tau distribution, white matter disconnection, synaptic dysfunction, vascular injury, other proteinopathies, and diminishing neural reserve, rather than CTE stage alone. Evidence that advanced CTE remains associated with dementia after excluding major comorbid neurodegenerative diagnoses strengthens its clinical relevance.[4] Nevertheless, exclusion of comorbidities does not establish that advanced CTE preceded dementia, and it creates a selected subgroup that differs from the frequent mixed-pathology presentation of older donors. Inverse probability weighting for donation likewise cannot resolve the temporal ordering of pathological progression and cognitive decline. The reported risk ratio should therefore be interpreted as an association between dementia and a high-stage terminal pathological state, rather than as an estimate of the independent effect of CTE on dementia. Analyses of stage-specific age-standardised absolute risks, non-linear age effects, dementia-to-death intervals, regional tau burden, and interactions with TDP-43, vascular, white matter, and Alzheimer-type pathologies would help distinguish CTE-specific associations from a broader multipathological syndrome. Prospective cognitive and neurophysiological assessment, longitudinal imaging and fluid biomarkers, and eventual neuropathological confirmation are needed to determine when pathological progression becomes functionally consequential. Bridging this pathology-to-function gap is essential before advanced CTE at death can be interpreted as the process that caused dementia during life. References 1.
